chore(plugin): bump to v0.1.3 + add CHANGELOG

Version bump to ship the user_id revert. Adds the missing CHANGELOG file for the plugin and back-fills 0.1.2 (which went undocumented) so users upgrading have a clear "what changed and how to recover stranded memories" entry.
